The goods and services shown on this price list are those we can provide
to our customers. You may choose only the items you desire. However,
any funeral arrangement you select will include a charge for our
services. If legal or other requirements mean you must buy any items
you did not specifically ask for, we will explain the reason in writing
on the statement we provide describing the funeral goods and services
selected.
This list does not include prices for certain items that you may ask us
to buy for you such as cemetery or crematory services, flowers, and
newspaper notices. The prices for these items will be shown on your
bill or statement describing the funeral goods and services selected.
We will not charge you for our service in buying these items.
INTRODUCTION
In the past funeral service pricing has been a relatively simple
process. Under new funeral guidelines outlined by the Federal Trade
Commission many of the costs previously associated with a funeral
service must now be listed separately. While this itemization of costs
does not necessarily affect the total cost of a funeral, it may make
selecting funeral services a somewhat more complicated process. In the
long run, however, it should enable families to better understand the
various expenses associated with a funeral. This booklet presents our
prices for various elements of our funeral service options. Our staff
will assist you with all of your funeral arrangements and will explain
all the information contained in this booklet.
In general, there are six categories of costs associated with a funeral service. These are:

to each of the above categories. Our staff will assist you in selecting
the services you desire.

FUNERAL SERVICE OPTIONS
For the convenience of our families we have designed several packages that include various items of funeral service into a single selection. These prices do not include cash advance items, caskets, outer burial container, cemetery charges, or shipping containers.
THESE PACKAGES HAVE ALREADY BEEN DISCOUNTED AND ANY CHANGES WOULD BREAK THE PACKAGES AND CAUSE EACH ITEM TO THEN BE INDIVIDUALLY PRICED WITH NO DISCOUNT.
Option 1. Traditional Funeral Service: 2,690.00
This option is designed for those families who wish a viewing before the funeral service, followed by a funeral service, and a committal service at a local cemetery. It includes use of facilities and equipment for viewing and funeral service, transfer of remains to the funeral home; embalming, dressing, cosmetology, casketing of remains, use of the necessary vehicles, needed funeral supplies such as memorial or mass envelopes, and Registration Package A.
Option 2. Immediate Burial: 1,350.00
Immediate burial applies in those instances that are without any ceremonies and burial occurs within 24 hours. This option of immediate burial includes transfer of remains to the funeral home, care of the remains until burial, casketing of the remains, and transportation to a local cemetery.
Option 3. Direct Cremation: 1,350.00
Direct cremation applies to those instances that are without any ceremonies and cremation occurs as soon after death as possible. This option of direct cremation includes transfer of remains to the funeral home, care of remains until cremation, use of necessary facilities, and transportation to a local crematory.
Option 4 Forwarding Remains to Another Funeral Home: 1,570.00
Our charge for forwarding remains to another funeral home includes the removal of remains, embalming,care of remains until transfer, use of necessary facilities, and transfer to St. Louis, MO or Springfield, IL airports.
Option 5 Receiving Remains from Another Funeral Home: 1,000.00
Our receiving service applies to those instances where death and preparation of remains has occurred outside our service area, and the casketed remains are transferred to our funeral home for burial without any funeral or graveside services. Our charges include transfer of remains from St. Louis, MO or Springfield, IL airport, care of the remains until burial, and transportation to a local cemetery.
CASH ADVANCE ITEMS
These are items you might ask us to purchase for you. We add these
items to your bill. Items such as newspaper notices, clergy gifts, etc.
are commonly called cash advance items. The specific items you desire
and the actual cost of these items will be detailed for you as part of
this funeral arrangement.
TRANSPORTATION
We do not charge extra for local transportation, which occurs within a 25-mile radius of our funeral home. If, however, we are called upon to use vehicles beyond this radius, our charge is $1.50 per mile, one way, per vehicle
ITEMIZED FUNERAL SERVICE PRICES
The funeral service prices listed below are those associated with the funeral service itself. The goods and services shown are those we offer our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral service goods and services selected.
Basic services 1,200.00
Our basic service charge includes, but is not limited to, staff to respond to the initial request for service; arrangements conference with family or responsible party; preparation and filing of necessary permits; placement of obituary notices; planning of funeral arrangements; coordination of services with cemetery, crematory, vault company and others required. Also included in this charge are overhead expenses relative to our facility, such as insurance, maintenance and utility expenses, secretarial and administrative costs and equipment and inventory expense. This fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select.
Embalming 420.00
Embalming includes the use of the preparation room and necessary chemicals. Except in certain special cases, Missouri law does not require embalming. Embalming is necessary if you select certain funeral arrangements, such as a funeral service or viewing with the body present. If you do not want embalming, you have the right to choose an arrangement that does not require embalming.

TERMS AND CONDITIONS
ASSIGNMENT TO US OF MONIES DUE: You agree that any monies assigned to us
in conjunction with this sale shall be paid to us within 60 days of the
date of this Agreement. Upon our giving you at least five (5) days
prior written notice that any monies due under the assignment(s)
described in this Agreement have not been paid to us as promised, we can
require that any such unpaid amount(s) previously credited to your
account be paid by you at once.
WAIVER OF RIGHTS: We have not, by the execution of this Agreement with
you, waived any rights we have to file a claim in the estate of the
decedent named in this Agreement. Any amounts collected by us from the
estate of the decedent, or from any other source, shall be credited
against any amount owed by you under this Agreement.
SELLER AGREES: We agree to furnish all services and merchandise as
indicated on the Statement of Funeral Goods and Services
Selected/Purchased Agreement.
WARRANTIES WE DISCLAIM: By signing this Agreement you understand that
we, not being the manufacturer of any of the goods being purchased,
expressly disclaim all warranties expressed or implied, of merchant
ability of fitness for a particular purpose or otherwise. The entire
risk as to the quality and performance of the goods remains with the
manufacturer and not us, will assume the entire cost of all necessary
servicing and repair. We are selling the goods on an as is or with no
fault basis. You agree that you are not entering into this agreement
based on any statement of fact or promise made by us to you which
relates to the goods bring purchased, on any description by us of the
goods, or on the exhibition of any sample or model of the good. You
also agree that the failure of the goods to conform to any such
statement of fact, promise, description, sample or model shall not give
you any right against us.
LIMITATION OF DAMAGES AND REMEDIES: by signing this agreement you
expressly waive and you agree that you shall not be entitled to recover
damages or losses of any kind, whether direct or consequential, based on
negligence. You further acknowledge and agree that emotional distress
will not be one of the claimed items of damage for any breach of
contract.
NOTICES TO YOU (THE BUYER): Notices to you shall be sufficient if mailed
to your last known address.
DELAY IN ENFORCEMENT: We can delay enforcing any of our rights under
this agreement without losing them. No such delay, forbearance of other
action or inaction on our part will create any new Agreement or
understanding with respect to the payment or other terms herein unless
there is a writing signed by both parties with respect to any such new
Agreement or understanding.
PARTIAL PAYMENTS: We can accept partial payments, even though marked to
indicate payment in full or full satisfaction of debt, without losing
any of our right sunder this Agreement. No acceptance by us of a check
for such partial payment shall be deemed a modification of this
Agreement.
COLLECTION COSTS AND ATTORNEYŐS FEES: This Agreement may be referred to
an attorney who is not our salaried employee for collection or
enforcement. In such event, you agree to pay, in addition to default
charges, all reasonable costs of collection, including court costs and
reasonable attorneysŐ fee incurred by us.
RESPONSIBILITY FOR PAYMENT: This Agreement shall not constitute a
release of liability imposed by law upon the decedentŐs estate. All
persons signing this Agreement will be responsible individually and
together, for paying all amounts due under this Agreement. We can take
action against you to collect amounts due under this Agreement, even if
we do not take such action against any person(s) signing this Agreement.
LIMITATION OF ACTION: You agree that to the extent allowed by law, any
action against us for breach of this Agreement must be commenced by you
or your representative(s) within one year after such cause of action has
occurred.
INVALID PROVISIONS: If any part of this Agreement is unenforceable, the
rest of this Agreement will stay in effect.
TITLES AND CAPTIONS: The titles and captions appearing at the beginning
of the various sections of this Agreement have been inserted for the
convenience of reference only. They will not in any way affect the
construction, interpretation or meaning of the text of said sections
themselves.
ENTIRE AGREEMENT: This Agreement contains all terms which have been
agreed upon by us and you relating to the goods and services listed on
the Statement of Funeral Goods and Services Selected/Purchased . This
Agreement replaces all other discussions and agreements, whether oral or
written, relating to those goods and services. No subsequent discussion
or agreement can change the terms of this Agreement unless it is written
and is signed by both of us.
JURISDICTION: This Agreement shall be governed by the local law of the
state in which the funeral service is being performed and you agree that
the jurisdiction of any suit involving this will lie in the courts of
the jurisdiction.
